SHUTTLE VAN PERMISSION FORM

Parents please complete the form at the link below to give LBBC staff and drivers permission to pick up your student on our Shuttle Van to come to church.

Our vans can pick students up from the school (after school), or we can pick up from your house. Parents, we have responsible adult drivers who are licensed and insured. Your students will be in great care from the time we pick them up until we drop them off at home. Of course we would love for you to join us at church too!

APPROVED AUTHORIZATION: By electronically signing below, you agree that you are the parent and/or legal guardian as listed within this form for the child who is listed within this form. As so, you give your consent and full authorization for the Lemoyne Boulevard Baptist Church Shuttle Van to pick up your student for church activities on Sunday morning or Wednesday night.

All student pick-ups will be confirmed with you on the day of pick-up by a church staff member or the shuttle van driver. The LBBC Shuttle Van may pick-up your student from school (after school) or from your address of residence.
